The primary goal of the progrm is to keep patients medically stable, prevent future hospitalization, decrease the incidence of noncompliance with medication orders, increase or maintain safety through fall risk assessment and removal of physical barriers, improve appetites, and prevent further degeneration of cognitive status. |
